2dcolidxrange <col> <idx> <idxend>
<col> is the editorcolor[] starting index. <idx> and <idxend> are the actual color index start and end.
Editor colors from <col> onward will be mapped to [<idx> ... min(<idxend>, 255)].
Takes precedence over '2dcol'.
